Before the visit:
If your insurer requires authorization or referrals please make sure to obtain them before the visit. Please make sure to check with your insurance to see if we are in network and if not in network please request an estimate in writing of the fees before the likely charges before the visit. If you need glasses prescription, make sure you are scheduled for one.
Duration:
If you are a new patient or if you require testing, cataract evaluation – then please plan to be there for about 2 hours.
For return patients, please allow 75 minutes, more time might be needed if additional testing is needed. Unlike other practices, we do what we can to fit specially returning patients in when they are experiencing problems which could delay appointments. However, most patients know that this means that if they have a concerning problem, we will try to fit them in. As we make sure to answer patients questions and are through in our care, visits can take longer at times. We thank you in advance for understanding.
What to bring:
- List of your medications, dosages and frequency
- List of your medical diagnoses, allergies, and procedures done on your eyes and elsewhere on your body
- Updated insurance cards
- Payments for services including copay or refraction (measurement for glasses prescription). We only do refraction upon request and not automatically. Kindly please let both the scheduling and intake technician know that you would like refraction before any drops are instilled into your eyes.
- Sunglasses
- The referring and primary care providers name and contact information so that we can communicate information back to them
- A driver if you are unable to drive with your eyes dilated
- A face mask
- If you have granted someone power of attorney, then please provide that information on the day of visit.
